What I love about these photos is the difference in how wide our patient is able to smile once he is comfortable with how he looks. Porcelain Veneers were placed to close the spaces between these severely worn upper teeth. We built back what had been worn away and created a more appropriate relationship to his lower teeth in order to minimize future wear due to excessive forces. Our goal in this case was to create a more harmonious flow of our patients' front teeth as well as obtaining improved color. At a later date, we may continue broadening his Smile by adding to his back teeth. We chose, at this time, to obtain the most with the least. Two worn front teeth were detracting from the smile and making her self-conscious - she just didn't like the way her teeth looked. Two CEREC Crowns were designed and placed in one visit! Tooth colored composite was initially bonded to the teeth at the beginning of the visit (a "mock-up") to allow the patient to see our vision for rejuvenating her smile. Patients are amazed when we hand them the mirror for their approval. It's nice to leave the same day with a new smile and new confidence. Our patients' concern was the wide range of shapes and colors of previously placed metal-based crowns that did not match his natural teeth. This was most apparent in the mismatch of his two central incisors - the teeth which primarily "center" the smile and set the stage. The goal was to minimize these differences, creating more visual harmony with a minimum of tooth replacement. If we were replacing 6-10 teeth, our freedom to design an entirely new smile would be much greater, however, the goal here was to do more with less. Our patient was very happy with the result ...
Having that inner confidence about your appearance is immeasurable. A comprehensive approach was required to achieve this beautiful result. Many of these teeth required root canal prior to placing new crowns. Please notice the loss of tissue between the upper front teeth known as the dreaded "black triangle" on the before close-up photo as compared to the after. One of the other areas we concentrated on, in this case, was to extend the continuity of her smile back into the face - this gives a broader more pleasing appearance and if you notice, how much more the eyes "pop" ...
These two front teeth were placed many years ago and splinted together in order to mask a large space. Our goal was to match the adjacent teeth and create a more harmonious appearance to the eye with CEREC crowns.
The shapes of the teeth were modified to fit proportionately and the result is a beautiful Smile that fits the face. A crown lengthening procedure was utilized to allow for properly sized and positioned teeth. The patients' existing porcelain-fused-to-metal crowns were replaced with Porcelain for a more vibrant and lustrous appearance. Having Teeth that match each other just looks better. Six Porcelain Crowns were designed to match our patients' face and outgoing personality. At a later date, the back teeth visible on the right side of the photo should be "built out" to be more symmetrical to what is seen on the left, resulting in a broader Smile.
This was a Full Mouth Reconstruction case, involving severe wear which resulted in what we call a "bite collapse". In order to achieve restored function and appearance, we elected to replace the patients' worn and broken Partials with new, satisfying our budgetary limitations. A Permanent Bridge was placed across the upper six front teeth and the bite was "opened". Note how much better the new teeth fill the available space. The change in appearance speaks for itself. Note the strain in the neck muscles when forcing a smile in the before photo as compared to the relaxed, natural smile ...
The patient felt that the large space between her front teeth was unattractive. Restoring proportionality to create the illusion of properly sized teeth was a challenge. Pre-positioning the teeth by orthodontic means would have made the case easier but added an element of extended treatment time that the patient was unwilling to undertake.
The main concern of this patient was that she did not want "big" teeth. The shape and the length was worked out mutually by adding tooth colored material to preview our proposed changes. Obviously, the teeth had been severely worn, due to a grinding habit, and a change of shape was necessary using CEREC crowns. An Occlusal Guard was fabricated for nighttime wear.
They say that one of the hardest things to do in Dentistry is to make a single front tooth veneer or crown and have it blend and look natural. I think this is a pretty good result. Of particular interest is how we were able to "grow" back the missing gum tissue between the front teeth simply by shaping the tooth correctly
